Patient Education for Disease Management

Living with Congestive Heart Failure: A unique program provided by UH Geauga Medical Center is Living with Congestive Heart Failure. The purpose of this program is to provide patients with the latest information on living well with congestive heart failure. Pharmacists review the basics of heart failure management so that patients may gain a better understanding of proper disease management. The discussion covers diet, medications, lifestyle, monitoring and action plans. Patients are given a booklet that contains the covered information as well as a request for a follow-up call.

Warfarin Education: Another excellent program offered at UH Geauga Medical Center provides patients with information on warfarin (Coumadin) or other anticoagulants as they plan to have orthopaedic surgery. With this program, our pharmacists will review the need for follow-up testing, what to monitor, diet, and lifestyle.

Sterile Products and Compounding

Sterile compounding is the process of preparing custom medications within a sterile environment. This process improves patient safety by allowing a very specific formulation of medications that takes into consideration any personal health issues, allergies, or specific treatment needs. The central pharmacy prepares these custom medications and meets all requirements of USP 797, the guideline authority on quality of compounded and sterile products. Products dispensed for inpatient use through the pharmacy include:

  • Chemotherapeutic preparations
  • Compounded oral medications
  • Sterile intravenous admixtures
  • Total parenteral nutrition preparations
