Senior Assessment Program Helps Secure a Healthy Future

The Senior Assessment Program at University Hospitals Geauga Medical Center in Chardon, Ohio, provides a comprehensive, multidisciplinary evaluation for senior adults. This complete assessment is used to identify specific issues in older adults, including:

  • Complex medical problems
  • Depression or anxiety
  • Difficulties performing daily activities
  • Medical management issues
  • Driving safety
  • Falls
  • Memory loss or confusion
  • Personality or behavioral changes

In addition, the Senior Assessment Program team can identify whether a senior adult can continue to live independently and, if necessary, make recommendations for safe and healthy living arrangements for the future.

Comprehensive Assessment

The multidisciplinary staff of the Senior Assessment Program includes the following highly skilled specialists:

  • Board certified physician
  • Clinical social worker
  • Geriatric psychiatrist
  • Occupational therapist (home evaluation)
  • Pharmacist
  • Physical therapist
  • Social worker

Working together with the one-day assessment, our team meets with the senior adult, referring primary care physician and family members to evaluate the patient’s needs and to make safe and effective recommendations for the future. In addition, the occupational therapist makes a home assessment to ensure personal safety.
