Anticoagulation Services

The Anticoagulation Monitoring Service at UH Harrington Heart & Vascular Institute provides a safe, efficient and unified approach to monitoring outpatient anticoagulant therapy. We work with the patient’s primary care provider or other physician specialist to ensure that he or she receives the appropriate follow-up care.

Anticoagulants, such as Coumadin®, Jantoven® and generic warfarin, are often used to treat or prevent blood clots in people with certain medical conditions. These conditions include deep vein thrombosis (DVT), pulmonary embolism (PE), stroke, atrial fibrillation, myocardial infarction (heart attack), or mechanical heart valve, recent knee or hip replacement.

Frequent and careful monitoring is required to ensure the patient is taking the correct dosage and to minimize harmful effects.

Services We Offer

  • Use of a finger stick to perform on-site blood testing
  • Dosing adjustments
  • Patient and family education
  • Screening and counseling for food and drug interactions
  • Home monitoring (may be available if certain criteria are met)
  • Reports to the patient’s physician

Physician Referral

A physician referral is required for the Coumadin Clinic. Patients or their physician may contact the clinic to request a referral form.

Referrals and orders can be faxed to 440-285-6110.

The referring physician does not need to be part of the University Hospitals health system.
