No problem with provider but I find this survey unnecessary every time I see my doctors. If you are going to ask questions it should be something like, was this appointment different for any reason other than normal? If so, how? What can we do to improve our service?
Been seeing Dr. Furness for years, he's always treated me right he's a great doctor I had an addiction with alcohol been sober for 14 months now and he's ask how I am doing every time I see him he's a really caring person
