Based on your health situation, we provide you with the appropriate level of medical care you need to maintain and improve your health, while remaining in the comfort or your own home.

Rest assured that you will receive the same high level of care and compassion at home that you would receive during a hospital stay or an outpatient visit. We coordinate your treatment with your doctor, involve you in the planning of your care, and keep you informed every step of the way to help you achieve an optimal outcome.

Home Care Programs Tailored to Meet the Needs of Each Patient

There are two types of home care programs we offer to patients, based on their medical situation and needs. These include:

Traditional Home Care

Following a patient’s hospital stay, traditional home care services are provided to help them continue the recovery process at home. We also help patients learn to live with a chronic illness or disability, with the goal of helping them regain their independence, become self-sufficient and maintain optimum health. Home care services may also be recommended for patients who have not been hospitalized, but whose health situation worsens or changes while at home.

Hospital-at-Home Care

This home care program allows certain eligible patients to avoid a hospital admission altogether. Potential patients are typically identified in the ER and offered the option of receiving inpatient-level care and monitoring in their home instead of being admitted to the hospital.
