Dementia Care Program

The Liberty Assisted Living at University Hospitals Extended Care Campus offers dementia care programs to residents who require additional structure and support due to cognitive, emotional and behavioral issues. The program is designed to address the needs of those residents who are no longer able to function independently with only occasional reminders but are not quite ready for a more restrictive long term care or locked dementia unit. This unique behavioral/social program facilitates quality functioning through structured, supervised and fully supported activities/events uniquely designed for the needs of residents with dementia.

Services include:
  • A personal escort to meals who will stay with the resident to support socialization and structure throughout the dining experience.
  • Residents enrolled in the program will participate in special memory and cognitive activities designed to enhance those skills that are still intact.
  • This unique program will allow residents to maintain their dignity and highest level of functioning with the support of caring professionals specifically assigned to them.
  • Participation in individual or small group visits to the multi-sensory snoezelen room, the therapeutic cooking, pet visits and music therapies and support and supervision while participating in large group events.

Goal: To provide residents with the early stages of dementia the opportunity to remain in the most independent environment as possible.

Criteria:
  • Short term memory loss that interferes with resident’s ability to get to meals with a simple reminder.
  • Unable to remember where and when activities are occurring as demonstrated by wandering around to find the location or forgetting about the activity after a brief interval of reminders.
  • Difficulties with initiation of tasks including socialization that prevents them from interacting with others and leads to isolation.
  • The presence of cognitive difficulties including poor insight, judgment and/or paranoia that tends to isolate the resident and/or leads to increased agitated behavior.
  • Behaviors and mood disturbances that interfere with the residents and others well being. Requires re-direction and structure to reduce symptoms.
  • The presence of a mental disorder that interferes with the residents’ ability to independently socialize with others and/or function in the mileau without significant structure and supervision.

Fee: Enrollment in the dementia care program is an additional $350 per month.
