Revised “Research Subject Rights” Phone Line in Consent Form

The consent template language has been revised to change the “Research Subject Rights” phone number that has been established through the University Hospitals Case Medical Center Chief Medical Officer.

The new number, which is 216-983-4979, is a dedicated hotline for subjects to call if they have any questions or concerns about participation in any research study conducted by UH staff, faculty or its affiliates, or approved by the UH Case Medical Center IRB.

The previous number, which is 216-983-5633 is still in service and subjects utilizing this phone number will still be able to discuss any research related concerns with the Chief Medical Officer’s Office.

The UH Case Medical Center IRB consent template and tutorial have been updated to reflect this change. Please be sure to use the newest version of the template for all new IRB submissions.
