Making Referrals to UH Harrington Heart & Vascular Institute’s Vascular Center
To faciliate expedient, efficient care, the Vascular Center offers referring healthcare providers convenient options for referring patients:
- Refer patients to our vascular physicians and surgeons through UH Rapid Referral. Once the referral is made, a patient will have an appointment within 48 hours. Then, when the diagnosis and best treatment options have been determined, we will provide referring physicians with the necessary information for further follow-up and treatment.
- Call Us: Contact 1-866-UH4-CARE to refer your patient, our call center representative will provide you with the date and time of his or her appointment.
Required Information
We ask for the following patient information to be available at the time of referral to facilitate the request:
- Full name
- Date of birth
- Social Security number
- Current address
- Current phone number(s)
- Insurance information
- Presenting symptoms
- Medical history
- Diagnosis
If using your own outpatient referral form, please make sure it includes the required information.
