Enrollment: Healthy Kids, Healthy Weight
Healthy Kids, Healthy Weight is open to children four to 18 years old, with a body mass index (BMI) at or greater than the 85th percentile.
Most children are referred to the program by their primary care doctor. Initial screening is done in the doctor’s office. When the doctor identifies a potential candidate, the family receives information about the program. If the family thinks it is a good fit, the doctor can provide a formal referral and direct the family to contact the program.
Parents or caregivers should call the Healthy Kids, Healthy Weight team at 216-844-1966. A team member will answer questions and send a registration packet to the family by mail.
The family should fill out the registration forms as completely as possible and return it to the hospital as directed.
Once the completed packet is received, a team member will contact the family to schedule an initial evaluation visit at University Hospitals Rainbow Babies & Children’s Hospital. The initial visit is designed to identify the root causes of the child’s weight issues and educate the family about potential long-term health effects.
At the evaluation appointment:
- The child receives a complete medical and psychological evaluation.
- The family meets the dietitian and exercise physiologist.
- The family learns more about the program and what to expect.
The primary care physician will receive a full report of the team’s recommendations. Once the initial evaluation is complete and the child is determined to be a candidate for Healthy Kids, Healthy Weight, the family will receive detailed scheduling information.
