The application deadline is October 1. Only applications that are complete will be considered for interviews. Notification for interviews will occur via email no later than October 8.
Required for Fellowship
- Board Eligible or Current Certificate in Pediatrics, Internal Medicine, or Family Practice.
- Applicants must be a U.S. citizen, Permanent Resident, have work authorization, or J1 visa status. The hospital does not support H1 status visas.
- The program will accept the universal application available on the AMMSM website.

All applicants will proceed through the match at NRMP.
Your application will be complete when the following are received:
- Universal application
- 3 letters of recommendation mailed directly by the author to the address below. One letter should be from your Residency Program Director.
- A complete list of all Sports Medicine related rotations completed
- Medical school diploma
- Residency diploma / Certificate of residency completion (if available)
- Copies of state medical licenses
- USMLE or COMLEX scores
- ECFMG certificate (if applicable)
- Medical school dean's letter
- Medical school transcript
