Cardiac Rehabilitation: Cardiac Rehabilitation consists of four phases specifically aimed at patients with different needs. It is designed to improve the quality of life for those with cardiovascular disease through education, diet and exercise. To begin the program you must have a physician referral. Create Your Weight – Adult Weight Management Program: This innovative weight management program encompasses nutrition education, physical activity and the role of behavioral therapy in weight reduction and weight management. The program begins with a free introductory session followed by a 9-week course. (The program requires a registration fee and is open to employees and residents of Geauga County only).
